For: Pregnant Women
School Nurses
Caregiver & household contacts of infants under 6 mos.
Children ages 6 months through 18 years
All children younger than 18 MUST have a parent or guardian present at the clinic
THREE CLINIC SITES/THREE CLINIC DAYS
PICK THE CLINIC THAT IS BEST FOR YOU **DO NOT REGISTER AT MORE THAN ONE CLINIC ** YOU WILL BE REMOVED FROM THE LIST IF YOU ARE ON TWO!
~ Monday, November 9th 5:30pm to 8pm in MANSFIELD
~ Wednesday, November 11th 9:30am to noon in COVENTRY
~ Friday, November 13th 5:30pm to 8pm in TOLLAND
